The Following Paperwork is required for All RavenStar Sessions

Age Verification Page:

This Page is Legally Verifying your Age as 18 or Under 18

If you are under 18 you MUST have a Parent or Legal Guardian sign their permission

Permissions Page:

This page gives RavenStar permission to Photograph you

This is where you decide whether or not RavenStar can or can not use your photos for Promotions

The Model Questionnaire is Optional

This Questionnaire allows us to better get to know your style, what you are looking to do in Modeling, any prior

experience. This gives us a good base to work off of.

The reason for all this paperwork is for legality, to cover US and YOU, in the event a situation should arise.

Once this paperwork is signed, it is good until you want to change anything. If you sign the Age Verification under the age

of 18, You MUST resign it when you reach the age of 18 for Legal reasons.
